  • You can avoid the $50 late fee by completing the Immunizations, Tuberculosis Screening, and Medical History sections of the Student Health Form before the end of the first week of class (add/drop week). Please see Student Health Records.

What services does the Student Health Fee include?

  • Physician, nurse practitioner, and physician’s assistant office visits
  • Acute care visits
  • In-office lab tests and diagnostics (including blood draws, EKGs, and basic X-rays)
  • Annual physicals
  • Chronic disease management for asthma, diabetes, auto-immune disorders, and other long-term conditions
  • Coordination of care with specialists
  • Preventive care visits
  • Nutritional and health coaching

Does the Student Health Center offer insurance?

  • No, the Student Health Center does not offer insurance at this time. The Student Health Fee is as a result of the benefit for health care services and not an insurance program. For more information, please visit the Student Health Center’s Insurance Questions web page.

How do I schedule an appointment with the Student Health Center?

  • During the academic year, appointments can only be scheduled in person for the current day, please keep in mind that they are scheduled on a first-come, first-served basis. During the summer, students may schedule an appointment via phone.

How many times can you go to the Student Health Center?

  • You can visit the Student Health Center an unlimited amount of times.

    I’ve had a TB test before, can I just submit those results?

  • Per our department policy, TB tests should be tested within 1 year.

In what instances would the Student Health Center charge me?

  • Some vaccinations, specialized lab procedures, or diagnostic testing may fall outside the scope of the services covered by the Student Health Fee. For more information, please visit the Student Health Center’s Health Center Charges web page.

What should you bring when you visit the Student Health Center?

  • Please bring your Flames Pass, a valid driver’s license, and an updated insurance card.

When did the Student Health Center implement the student health fee?

  • The Student Health Center implemented the health fee in the Fall semester of 2016.

Where can I drop off my immunization records?

  • You may drop off your immunization records at the Student Health Center & Wellness Initiatives department which is located in Commons 3, A wing.

Do I need to complete my Financial Check-In before I request for a student health fee exemption?

  • No, you may request for a health fee exemption now. However, you must complete Financial Check-In (FCI) and pay the student health fee in order for it to be removed from your account. When your FCI is complete and your request is approved, you will be refunded.

What forms of immunization records will be accepted?

  • The preferred method is for you to fill out the Liberty provided health form with the doctor’s signature and the dates of your vaccinations located in your ASIST account. However, there are other acceptable methods:
    • Official Department of Health issued document
    • Document with the doctor’s signature, emblem, stamp, or seal is present
